Application Method
You may submit your requests regarding your rights listed in Article 11 of the Law No. 6698 on the Protection of Personal Data (the "Law") to Ünlem Bilişim Teknolojileri A.Ş. by one of the methods described below, pursuant to Article 13 of the Law and Article 5 of the Communiqué on the Procedures and Principles of Application to the Data Controller. You may apply using the form below or in free-form writing; using the form is not mandatory.

Information Your Application Must Contain
Under Article 5 of the Communiqué, whether you use the form or apply in free-form writing, your application must contain:
- Name, surname and, if the application is in writing, signature,
- For citizens of the Republic of Türkiye, the T.R. identity number; for foreigners, nationality, passport number or identity number, if any,
- Residential or business address for notification,
- E-mail address, telephone and fax number for notification, if any,
- Subject of the request: which of your rights under Article 11 of the Law you wish to exercise and an explanation of your request,
- Your relationship with our company (e.g. website visitor, person who filled in a form / prospective customer, customer, business partner, job applicant, office visitor) and any information and documents related to your request.
Your application will be concluded free of charge as soon as possible according to the nature of the request and within 30 days at the latest. If the operation requires an additional cost, the fee in the tariff determined by the Personal Data Protection Board may be charged. We may ask for additional information to verify your identity.

As the "Talep Konusu" (Subject of the Request) section of the form has no writing space, you may write your request on an additional page. If none of the options under "Şirketimizle ilişkiniz" (Your relationship with our company) applies to you (e.g. website visitor or person who filled in a form), tick "Diğer" (Other) and describe your relationship.
